Shepparton teenager Jack Brophy's jaw was broken in three places and will require surgery, but Echuca United Football Club will appeal the penalty.
NSW police officers were called to the match at Tocumwal, just over the NSW-Victorian border.
Footage of the incident is expected to be viewed as part of the probe.
Congupna was thrashing Echuca United when the melee broke out in the dying minutes of the game.
Congupna coach Bruce Fuller said it was a "pretty disgusting" incident.
Mr Fuller said Jack had gone to help a teammate.
"A bit of a melee broke out between about 10 or a dozen players . . and a kid ran in from probably 20m and king-hit him from behind," he said.
"The force of the blow knocked his mouthguard probably 10m out of his mouth.
"The kid who hit him got everything he deserved. He's gone missing. They can't find him."
Jack's mother, Carmel Brophy, said she was shocked.
"I couldn't believe it," Ms Brophy said.
The player who hit Jack was suspended for 200 weeks after he failed to appear at the tribunal to answer the charge.
Another player was suspended for 32 weeks on two counts of striking and one count of kicking and a third was suspended for 12 weeks on two counts of striking.
Echuca United president Max Prendegast said the club was disappointed and would take its own action against the players.
"The club also believes a suspension of 200 weeks is excessive for a 15-year-old and has lodged an appeal to the VCFL against the player's sanction only, seeking a more appropriate penalty, while still recognising the seriousness of the incident," he said.
